The FDA cleared Moderna’s mFlusiva on 5 August 2026, making it the first influenza vaccine built on messenger RNA technology to reach the market. The approval completes a turbulent regulatory journey and opens a new chapter for a platform that changed vaccination during the pandemic.
Influenza kills between 290,000 and 650,000 people worldwide every year, according to the World Health Organization. It hospitalises millions more, disproportionately affecting adults over 65, who account for around 57% of flu hospitalisations and 71% of flu deaths in the United States in an average season. For decades, the standard approach to preventing it has relied on egg-based or cell-based vaccine manufacturing methods developed in the mid-twentieth century, producing shots whose effectiveness varies considerably from year to year depending on how well the chosen strains match what actually circulates.
The approval of mFlusiva changes that picture. Not dramatically, and not overnight. But meaningfully.
What the FDA Actually Approved
The FDA approved mFlusiva (mRNA-1010) for adults aged 50 years and older, supported by a unanimous nine-to-zero recommendation from the Vaccines and Related Biological Products Advisory Committee, the panel’s first review of a new vaccine application since May 2023.
The approval is not uniform across the age range. For adults aged 50 to 64, the FDA issued a standard full approval, supported by clinical efficacy data from the FLUENT Phase 3 trial. For adults 65 and older, it granted an accelerated approval, a pathway that permits licensure based on a surrogate endpoint reasonably likely to predict clinical benefit, with a required postmarketing confirmatory trial. That distinction matters: the older group, who stand to benefit most from improved flu prevention, receive the vaccine under a conditional arrangement whilst further effectiveness data are gathered.
Moderna received the FDA’s approval letter on 5 August 2026, the day its Prescription Drug User Fee Act target action date expired. mFlusiva is Moderna’s fourth FDA-approved product, joining Spikevax and mNEXSPIKE for Covid-19 and mRESVIA for RSV. The company says it expects the vaccine to be available in select US retailers within weeks, in time for the 2026 to 2027 respiratory virus season.
What the Trial Data Show
The approval rests primarily on results from the FLUENT trial (NCT06602024). The Phase 3 double-blinded, active-controlled trial enrolled 40,805 adults aged 50 and older across 301 sites in 11 countries during the 2024 to 2025 influenza season. RT-PCR confirmed, protocol-defined influenza-like illness was observed in 411 of 20,179 recipients of mFlusiva and 557 of 20,124 recipients of the standard-dose comparator, corresponding to a relative vaccine efficacy of 26.6% (95% confidence interval, 16.7 to 35.4), meeting the criteria for both noninferiority and superiority.
That figure requires careful interpretation. It does not mean the vaccine prevented roughly a quarter of all flu cases. It means it performed 26.6% better than the standard-dose flu shot it was tested against, in the specific conditions of the trial. Efficacy in adults aged 65 and older specifically was 27.4%.
For the over-65 age group, accelerated approval was based on a separate immunogenicity trial enrolling 2,992 adults, in which mFlusiva triggered stronger antibody responses than a high-dose flu vaccine across all four influenza strains. A postmarketing trial will need to confirm clinical benefit in this group before full approval can be granted.
Comparative effectiveness against high-dose or adjuvanted vaccines, which are preferentially recommended for older adults in the US, remains undefined. Durability across multiple seasons is also unproven, given that the data come from a single influenza season. These are legitimate scientific limitations that will need to be addressed through the post-marketing trial and future seasonal data.
On safety, solicited adverse events were more common with mFlusiva than with the standard-dose comparator, including injection-site pain in 65.8% of recipients, fatigue in 45.1% and headache in 37.8%. Events were predominantly mild to moderate and transient. No new safety signals were identified across the Phase 3 programme and no cases of myocarditis or pericarditis occurred. The higher rate of local reactions is consistent with the reactogenicity profile seen across mRNA vaccines generally and was judged acceptable by the FDA’s advisory panel.
The Phase 3 efficacy results have been published in the New England Journal of Medicine (Leroux-Roels et al., NEJM 2026; 394:1803-1813; DOI: 10.1056/NEJMoa2516491), providing the peer-reviewed scientific record underpinning the regulatory decision.
The Regulatory Saga Behind the Approval
The path to approval was anything but straightforward. On 3 February 2026, Vinay Prasad, then director of the FDA’s Center for Biologics Evaluation and Research, issued a rare refusal-to-file letter, overruling the recommendation of the agency’s own career staff and declining to review Moderna’s application. Prasad argued that the FLUENT trial’s use of a standard-dose comparator rather than a high-dose or adjuvanted vaccine did not represent the best-available standard of care for adults 65 and older in the United States. The refusal did not cite any safety or efficacy concerns.
Moderna publicly contested the rejection, noting that CBER had approved the study design 18 months earlier and had permitted the trial to proceed as planned. The decision drew immediate and sharp criticism from clinicians, scientists and within the agency itself. Within approximately one week, after Moderna disclosed the FDA’s decision and its own displeasure publicly, FDA Commissioner Marty Makary reversed the decision, characterising it as “part of a conversation that occurs with all drug makers.” The reversal was reached through a Type A meeting, the FDA’s mechanism for resolving urgent disagreements, in which Moderna proposed the age-stratified regulatory pathway that ultimately formed the basis of the approved application.
Both Prasad and Makary have since left the FDA. Prasad departed at the end of April 2026; Makary also resigned amid the wider controversies over vaccine and rare-disease drug reviews earlier this year. Acting Commissioner Kyle Diamantas has since sought to steady the agency and rebuild industry confidence. The approval itself was ultimately made under acting leadership.
The episode unfolded against a wider backdrop of political pressure on mRNA technology. Health and Human Services Secretary Robert F. Kennedy Jr. had previously cancelled 22 mRNA vaccine development projects funded through BARDA, worth approximately $500 million, in August 2025.
Why mRNA Offers a Genuine Advantage for Flu
The scientific case for an mRNA-based flu vaccine rests substantially on manufacturing speed. mRNA manufacturing can compress production timelines from approximately six months, as required by conventional egg-based methods, to two to three months, potentially improving strain matching and enabling a mid-season response if a poorly matched formulation is identified.
The World Health Organization convenes influenza experts each year to study circulating strains in the southern hemisphere in order to select which strains should be targeted in northern hemisphere vaccines. That analysis is reliable but not always accurate, and can lead to mismatches producing weak vaccines, with efficacy ranging from around 50% to 60% in a good year to as low as 20% in a poor one. A faster manufacturing platform reduces the window in which such mismatches can occur.
The approval also carries an immediate implication beyond the flu vaccine itself. Moderna withdrew its US application for mRNA-1083, a combination Covid-19 and influenza vaccine, in May 2025 after the FDA requested flu component efficacy data, stating it would resubmit once FLUENT Phase 3 results were available. That data now exists. Moderna has said it plans to resubmit the US application, though no specific timeline has been announced. The combination vaccine was already authorised in the European Union in April 2026 under the name mCOMBRIAX, making it the world’s first licensed Covid-and-influenza combination vaccine. The mFlusiva approval also provides the Phase 3 clinical data embedded in Moderna and the Coalition for Epidemic Preparedness Innovations’ H5 pandemic influenza licensure plan.
A Crowded Market and an Uncertain First Season
mFlusiva enters a flu vaccine market that is already competitive. Adjuvanted and high-dose vaccines are currently preferentially recommended for older adults by the CDC, and head-to-head data against those products do not yet exist. Higher reactogenicity than comparators may influence uptake in older, multimorbid populations.
The access picture for the 2026 to 2027 season is complicated by an unprecedented legal situation. The CDC’s Advisory Committee on Immunisation Practices, the body that translates FDA approvals into clinical recommendations and triggers mandatory insurance coverage under the Affordable Care Act, is currently unable to issue new recommendations. A federal district court in Massachusetts stayed the reconstituted committee in March 2026, nullifying all post-June 2025 ACIP votes, after ruling that the committee’s reconstitution likely violated the Federal Advisory Committee Act. The federal government’s appeal remains pending before the First Circuit Court of Appeals.
Because mFlusiva carries no standing ACIP recommendation, the mandatory zero-cost insurance coverage trigger under the Affordable Care Act has not fired. Private insurers are not currently required to cover it without cost-sharing, though many may choose to do so voluntarily. Adults considering mFlusiva this autumn should confirm coverage terms with their insurer before scheduling vaccination, and should not delay getting any flu shot whilst waiting for the situation to resolve.
Additionally, much of the flu vaccine purchasing for the 2026 season was finalised by health systems and pharmacies before the approval, so initial physical availability may be more limited than the FDA decision alone suggests.
The Longer-Term Significance
mFlusiva is not a universal flu solution. It is the first approved product of a platform that has demonstrated it can outperform standard-dose vaccines in a rigorous Phase 3 trial, manufactured at a speed that conventional methods cannot match, and approved by the FDA despite considerable political headwind.
The confirmatory data required for full approval in the over-65 age group, the head-to-head data against high-dose and adjuvanted vaccines, and the post-marketing outcomes evidence across multiple seasons will collectively determine whether mFlusiva fulfils the longer-term promise of the platform. The science has cleared its first major regulatory hurdle. The clinical and commercial questions that follow are the ones that will matter most.
